Join us for an engaging, peer-led session exploring how councils and health partners can work together to bring healthcare closer to communities through neighbourhood-based models of care. This Innovation Exchange will focus on the practical opportunities and challenges of integrating services at neighbourhood level, and how place-based collaboration can support better outcomes for residents.
We’ll hear from Claire Kennedy, Joint Chief Executive at PPL, who will share insights from their work supporting systems, places and neighbourhoods to implement neighbourhood health models. Claire will explore how neighbourhood health both depends on strong local government leadership and creates new opportunities to deliver shared priorities across health, care and communities.
